From the app — some music apps export a playlist themselves. Look in the playlist's ⋯ menu, or under File → Export. Text, CSV and XML all work.
From a converter — if your app can't, a free playlist converter will. Search for export playlist to CSV. Make the playlist public first: converters can't see private ones. They run in a phone's browser too.
Everything you've streamed — many services will send you your listening history from their privacy page. If the download has a daily play-history file, use that one. It's small and already counts your plays.
